我是中国（江苏）第32期援桑给巴尔医疗队副队长，目前已经圆满完成在奔巴岛的医疗援助任务。
      还记得我拖着沉重的行李箱刚出机场，看到迎接的奔巴政府官员、驻点医院管理人员，他们用特有非洲口音英语跟我交流，那时我的心情是紧张忐忑的，这是些许对未知环境的恐惧，也是自己对冲破舒适区带来的不确定性的一种迷茫。转眼之间，我的援外生涯已经结束。如今，我能淡定从容地用英语和当地人交流，偶尔还能加上斯瓦希里语，和队友们一起精诚团结解决各种问题，完成各项医疗队任务。
      桑给巴尔家庭规模大，子女较多，儿童扁桃体、腺样体肥大发病率高，整个奔巴岛只有中国医疗队驻扎的阿卜杜拉姆才医院才能开展此类手术。阿卜杜拉姆才医院共有两个手术间，只有一台麻醉机适合小儿麻醉，没有麻醉恢复室，麻醉医生偏少。针对此困境，中国医疗队与当地医生协商，采取手术营的形式，连续两天，集中手术，一次完成40例患者。
      扁桃体手术须做气管插管全醉。儿童气道脆弱，气管插管易损伤小儿的呼吸道。咽喉部手术与麻醉共用一个空间，围术期易发生脱管、误吸、喉痉挛和支气管痉挛、术后躁动等并发症。这就给围术期麻醉的管理带来极大难度。术前，我与当地麻醉医生因地制宜，寻找可用仪器设备，充当工程师，进行组装，临时组建麻醉恢复室，确保患者麻醉复苏安全有效，缩短周转时间。针对一台麻醉机不能用于小儿的问题，我们采取手控模式，确保患儿各项麻醉参数、生命体征平稳。围术期我们与中国医疗队耳鼻喉科医生钱剑领导的手术团队配合默契、有条不紊，先后三次安全高质量地完成了全部120例小儿扁桃体、腺样体切除术，术后无一例并发症。
      作为队长，我一次又一次主持抢救了产后大出血、骨科严重创伤性骨折、普外科感染性休克等危重患者，常常面临“麻醉要上，手术要做，病人要活”，这种三难境地，我用丰富的临床经验带领医疗队的队友们一次又一次渡过难关，抢救了一个又一个非洲人民的生命。
这一年来，我和队友们用自己的技术和敬业精神赢得了当地人民的尊重和认可，同时也感受到他们的淳朴与热情，感受到他们对我们的包容和帮助，我们也真正地融入了他们的生活。

 I am vice captain of the 32nd medical team sent to Zanzibar from China (Jiangsu), and I have successfully completed the medical aid mission on Pemba Island.
  I still remember when I dragged a heavy suitcase just out of the airport and meet the government officials and  hospital management on Pemba, at that time my mood is nervous  and uneasy, this is a little fear of the unknown environment, and to break through the comfort zone of uncertainty of a kind of confusion. In a twinkling of an eye, my career of foreign aid was over. Now, I can calmly communicate with the local people in English, occasionally add Swazili, and unite with my teammates to solve various problems and complete the tasks of the medical team.
 People on Zanzibar usually have  a large family and many children. There is a high incidence of tonsil and adenoid hypertrophy in children. Such operations can only be performed at Abdulla Mzee Hospital, where the Chinese medical team is based. There are two operation rooms in Abdulla Mzee Hospital, only one anesthesia machine  suitable for pediatric anesthesia. There is no anesthesia recovery room and few anesthesiologists in hospital. In response to this dilemma, the Chinese medical team negotiated with local doctors and took the form of surgical camp for two consecutive days to complete 40 patients at a time.
 Tonsil surgery must be done under endotracheal intubation and general anesthesia. Because Children's airways are fragile, endotracheal intubation is easy to damage the respiratory tract of children.  Complications such as removal, aspiration, laryngospasm, bronchospasm and postoperative agitation are common in the perioperative period. This brings great difficulty to the management of perioperative anesthesia for throyngeal surgery and anesthesia sharing the same space. Before surgery, I and local anesthesiologists adapted measures to local conditions, looking for available equipment, acted as engineers, assembled, and temporarily set up the anesthesia recovery room to ensure the safe and effective recovery of patients' anesthesia and shorten the turnover time. In view of the problem that an anesthesia machine cannot be used in children, we adopt the hand control mode to ensure that all anesthesia parameters and vital signs were stable. During the perioperative period, we cooperated with the surgical team led by Qian Jian, an otolaryngology doctor of the Chinese medical team, and completed all 120 cases of pediatric tonsil and adenoidectomy safely and with high quality for three times, without any postoperative complications.
 As a captain, I hosted again and again to rescue the postpartum hemorrhage, orthopedic severe traumatic fracture, general surgery septic shock and other critically ill patients, often face "anesthesia, surgery to do, patients to live", the difficulties.I led medical  teammates through with rich clinical experience again and again, saving the lives of the African people.
This year, my teammates and I won the respect and recognition of the local people with our own technology and professionalism. At the same time we feel their simplicity and enthusiasm, feel their tolerance and help to us, we  also really integrated their lives.
